Medicine is confusing enough. The label shouldn't be.

FormVital turns a prescription label into a few short steps, written in the language you actually think in.

What the bottle says

TAKE ONE (1) TABLET BY MOUTH TWICE DAILY WITH FOOD FOR 7 DAYS. DO NOT SKIP DOSES.

  1. Take 1 tablet. Swallow it with water.
  2. Take it 2 times a day — morning and night.
  3. Eat something first. Not on an empty stomach.
  4. Keep going for 7 days, even if you feel better.
Are you taking any other medicine right now?

Example only. This is not real medical advice.

Reading a label is not the same as knowing English

Someone can order coffee, hold a job, and raise a family in English and still stall on take on an empty stomach or do not exceed. Pharmacy language is dense, abbreviated, and written for the pharmacist as much as the patient. When it isn't understood, doses get missed, doubled, or stopped early.

Short, not long

General chatbots answer with paragraphs. We answer with numbered steps you can read while holding the bottle.

It asks back

Allergies, other medicines, side effects you're already feeling — the questions a pharmacist would ask before handing you the bag.

Your language, first

Not English with a translate button bolted on. The steps are written to be plain in the language you read them in.

Three steps, start to finish

  1. Show us the label

Photograph the bottle or the printed instructions, or type them in by hand if that's easier.

  2. Answer a few questions

What else you take, what you're allergic to, how you've been feeling since you started. Nothing longer than a sentence.

  3. Get your steps

A short list in your language — when to take it, with or without food, what to watch for, and when to call your doctor.

Starting with five languages

These are the launch languages. More get added as we find speakers who can check the wording — a translation nobody has read out loud to a real person isn't finished.

  • Spanish Español
  • Chinese 中文
  • Arabic العربية
  • Hindi हिन्दी
  • Korean 한국어

FormVital explains what your prescription already says. It does not decide your dose, change your treatment, or replace your doctor or pharmacist. If the steps here don't match what you were told, trust the person who prescribed it and call them.

We're early, and that's the useful part

Our translation pilot is live and invite-only. Request access to try it and share feedback as we improve it.

Speak one of our languages?

Read our draft instructions and tell us where they sound wrong, stiff, or unclear. This is the work that matters most.

Teach ESL or work in a clinic?

Tell us what your students and patients actually get stuck on. We'll build around real confusion, not guessed confusion.

Want to support it?

Running the system costs money as more people use it. Grants, small donations, and hosting help all go straight to that.
